Is there a practice release form?

I understand that my daughter needs permission from her current coach to practice with another team during the month of May. Is there a specific form for this purpose?
The "Guest Player Release Form" on the NTSSA web site looks like it is geared toward participation in tournaments...

Neither my coach nor manager, nor the coach or manager of the team I am looking at, have seen a "practice release form" - separate from the "Guest Player Release Form".
There certainly doesn't appear to be anything like that on the NTX Soccer Forms site.

We used the Guest Player Release form and modified it to state practice with XXX team. If the coach is willing, you could leave it more generic by stating that the coach approves practicing with other teams from May XX through June XX, with the statement that it is for practice only.

Thanks all. NTSSA confirmed that there is not an official form for practicing with another team. I drafted a short permission letter based on NTSSA Bylaw 3.10.1. Coach posted the link to the bylaw above.

This also seems like it would meet the requirement in Bylaw 3.10.1.

That's what we've always done too.. just modified the Guest Player Form, since it already has most of the required info.
